It's now official. I'm scheduled for surgery on 9 May 12. The orthopaedist will open up the foot, peel back internal muscles, shave the bottom of the cuboid bone (with a chisel) to, hopefully, reduce the possibility of future ulcers, and try to stabilize the foot using internal fixators.

I'd still like him to provide me with a loaner and fix the foot in the shop for later pickup.

Mike this is my single post to your thread.Because of my recent exposure to the medical system dealing with a primary doctor and numerous specialty doctors,my suggestion to you is for you to make very clear what is your goals and then ask for what is a reasonable time span to achieve those goals.No B.S. Then hold them to that.

Better yet, get him to give you an affordable new one then rebuild the old one so you could keep it
for a backup.
